Its called "Remoter:Remote Desktop (VNC)" I just looked and its on sale for $1.99 right now.
If its on the same wifi network as your desktop it will automatically find it. To connect from a remote location you will need to do a couple of things first. None are particularly difficult.
1)You need to give the desktop you want to access a static IP so that it never changes.
2)You need to go to system preferences and enable "screen sharing" in the sharing preferences.
3)You need to enable remote access on your router. This will vary based on your router. I have an Airport Extreme and it is very simple. You simply go to the port mapping section and add "Apple Remote Desktop" to the allowed port forwarding. The remoter app uses the default port so all you have to do is change the local IP address that the router forwards the port to match the static IP that you gave your desktop.
Now, when you run remoter, you will have it look for the public IP of your router. When your router gets this request on that port, it knows to forward that request to the static IP that you gave your desktop. Since you've enabled screen sharing, you can now have full control over your desktop from anywhere in the world. The app will automatically handle resolution so it actually works very well even over 3g. I have mine setup so only an administrator of the computer can access it, which means you will have to setup your desktop user id/password in the remoter app, this way, not just anyone can connect to and control your computer.
