I have a 2000 5spd with 113xxx on it (the valves were just adjusted). I am aware that the noise the engine makes on a cold start is normal. However I started to notice a noise similar to the startup noise at highway speeds. I can hear it when I hold the gas steadily (cruising around 2-2.5k rpm) and it goes away/becomes less audible when I take my foot off the gas.
Based on search it appears it could be piston slap, engine knock, rod bearing, etc. I am sort of at a loss (the Honda dealer said it was normal because it is an old car after they charged me $400 not to fix it). When I first bought the car it had 5w 30 in it and did not make the noise at Highway Speeds (in fact I may have first noticed after putting 0 20w in it). I am thinking the next step is to try 5w 30 or 5w 20 and see if the noise goes away. If it does I am set otherwise I will keep searching.
It made the noise prior to the valve adjustment. Not sure if they increased the timing. I also put one tank of 93 through it and that did not make a difference (though it wouldn't hurt to try again because I also put fuel injector cleaner in that tank).

Guys, "knock" as in detonation doesn't sound like a knock. It sounds like someone ringing the cylinder head with a very small hammer.
I'm guessing that you're probably reading into the noise too much. Buy a mechanic's stethascope from Harbor Freight for $4 and try and locate the noise.
Could also be CV joints?

I know the owners manual of my 03 MT Insight calls for "premium" fuel. I have always used the cheapest grade of fuel available and have never had any "knocking" issues. Im guessing the majority of Gen 1 owners use cheap fuel as well. Has anyone ever had negitive effects with this and had to use the higher grades of fuel? Or is there a reason Honda calls out for "premium" grades of fuel?

The timing is not serviceable - i.e. it cannot be manually set. It's completely computer controlled
