2001 CVT shudders on starts

My 2001 CVT automatic has 90,000 miles on it. It has been surging on the freeway, usually at 70 MPH and uphill, for years. It also shudders when I start from a stop, rather it is in auto shut of or not. I first noticed the shuddering at about 25,000 miles, since then it has become much worse. I have had the transmission fluid changed at a Honda dealer at the recomended intervals, 30,000 & 60,000 miles. Does anyone know if it is fixable or just an annoyance I should put up with.

Trans or EGR?

The surging is only at 70 MPH or maybe at higher speeds. It happens when I drive a steady speed and go up a hill or sometimes just into a head wind. It happens maybe once every 5 minutes. When it does it, it feels like the car decelerates but the tach speed increases. That makes me think it is the transmission, but who knows?
