Cutting the stock speaker grille is not hard with a razor knife, though it is a little time consuming. Good speakers used to have big permanent magnets on the back, which made it harder to fit them into the doors because of the plastic and metal behind the OEM speakers.

There are now lots of low-profile speakers with relatively small-sized neodymium magnets on the market, so it's not that hard to find good speakers that will still fit inside the door.

Boston Acoustics offers a good low and high end option.

I use Alpine 2-ways in the rear to just suppliment the fronts. I did use them in the front before I got some Infinity Kapps 3-ways but the bass of the Infinities is just amazing. Cutting the rear carpet away is pretty easy but getting all the stuffing away can be tricky.
