Which bearing might this be?
If it's a complex repair at the shop, should they preventatively fix anything else at the same time?
Also, how long is it OK to drive with this?

at 147,000, I just started hearing / feeling thumping & similar noises & vibration. I went away sometimes, now it's gotten more severe.

Happens coasting or driving, changes with driving, braking / accelration, shifting. I can't imagine it's not a Bearing.

this is Gen1 MT, '01.

I have a choice of 2 shops: dealer or independent shop which usually works on fancier cars but has treated me well. I don't see many Hondas there.
Dealer sees very, very few Gen1 Insights, like: 1, but it knows Hondas. Last time they ordered the wrong parts for exhaust repair & something else.

I know 'almost' nothing about G1 Insights, but from I do know, this definately sounds like bad engine/transmission mounts. I'm sure someone will chime in and tell you exactly but from what I have read that sure does sound like it.

From what you describe it could be a variety of things. Maybe a combination of bad rear motor/transmission mount plus one other item - front axle, tires? Probably not a wheel bearing - those tend to have a rather specific kind of sound when they go, a whine/hum, some vibration, changes when you turn (if front wheel bearing)...
